Building on my writing and research to date, I explore how health care in high-income countries can better appreciate how people and communities define their health, and respond accordingly. It includes reviewing often-overlooked evidence, sharing the work of courageous community-oriented practitioners, and commenting on work increasingly being described as ‘community health’ or the ‘social determinants of health’.
